Survey block is Likert style and wouldn’t let me change the verbage or number of options. This is Block > Form. Delete the default fields in Edit Form Fields, add Field > Radio to be able to delete the last default field. Pick one of the questions and Duplicate it so that the questions are all part of the same form. Still have to configure what happens when they click submit, both the interface and the collection of data
